- Karai was created by Kevin Eastman and Peter Laird where she made her first appearance in Teenage Mutant Ninja Turtles v1 #53 (2003).

- In Teenage Mutant Ninja Turtles, Karai appeared in the setting of the 2003 animated television series where she was voiced by actor Karen Neil.
- In Teenage Mutant Ninja Turtles, Karai appeared in the setting of the 2012 animated television series where she was voiced by actor Kelly Hu. It was revealed that she was actually Hamato Miwa who was the daughter of Hamato Yoshi who was believed to had been killed by the Shredder. However, in reality, Oroku Saki had abducted the child and raised her as Karai. It was shown that during this time she formed a close friendship with fellow ninja Shinigami.

- In TMNT, Karai appeared in the setting of the 2007 animated film where she was voiced by actor Zhang Ziyi.
Video games
- In Teenage Mutant Ninja Turtles 2: Battle Nexus, Karai appeared as a boss and unlockable character in the setting of the 2004 beat 'em up video game where she was voiced once again by Karen Neil.
