Bane (Batman Films)
Bane, alias Antonio Diego, was the sole surviving subject of the mad Dr. Jason Woodrue's experiments, Diego was given a large dose of the super-steroid known as Venom, which transformed him into the mindless hulk of muscle which Dr. Woodrue dubbed Bane. After Woodrue's death at the hands of Poison Ivy, Bane followed Ivy around and did her bidding, eventually aiding her in her and Mister Freeze's plan to freeze Gotham City.
Although Bane was incredibly powerful, he was not indestructible: The source of his power lay in the tube attached to the back of his head that kept the Venom dosage flowing at regular intervals. Robin and Batgirl both realized this and quickly defeated Bane by yanking the tube out, cutting off the flow of Venom and causing the hulking monster to revert back into Antonio Diego. Diego's fate after this is unknown.
