Biography
Miho (Japanese: ミホ) was a mute Japanese woman who came to reside in Basin City.
Three years ago, she was involved in a fight with Tong gangsters where she managed to kill three of them by herself. However, she was nearly killed by the remaining two who had her in point blank rage. She survived only by the intervention of Dwight McCarthy who save her. As a result of this incident, she came to owe a life debt of honor to Dwight who was one of the few males she had a genuine respect towards. Because of this incident, she owes a life debt of honor to Dwight, and subsequently he is the only male character she is ever shown to have any non-lethal interaction with, even taking orders from him at one point. It would appear they have some sort of friendship, as he is also the only character she ever shows any physical affection towards. After Dwight is shot multiple times and he discovers Ava Lord has been manipulating him to commit murder, he flees to Old Town. He seeks refuge from Gail, and in order to gain the help of the Girls of Old Town, he tells the story of saving Miho's life from Tong gangsters. As according to honor, the girls are obligated to help him in his plan for revenge. Miho is instrumental during the fight with Manute at Ava Lord’s estate, stopping him from reaching Dwight and crucifying him through the arms with her twin swords. This leads Manute to have a great disdain for the Old Town girls.
Miho was seen stamping on a man while he is down in an alley as Jim tries to escape The Colonel who is stalking him. Her murderous actions causes a possible war between the police and the girls of Old Town, when she slaughters a cop, known as Jackie-Boy. Dwight then discovers that Jack was a police officer. As an attempt to prevent a war between the Old Town girls and the police, Dwight tries to dispose of Jack’s body in the tar pits. Upon arrival, he is attacked by some Irish mercenaries that are sent by Wallenquist. During the fight, Dwight accidentally sinks into the tar, and prays to God not to let him die. Miraculously, he was saved by Miho, who manages to pull him with her foot attached to a chain at the back of a car and she drags him just in time from the pits.
Miho patrolled the rooftops of Old Town as a silent guardian, watching for any intruders who might threaten the women under her protection. When Jackie Boy and his men entered Old Town and harassed one of the working girls, Miho intercepted them. She decapitated one of the men with a single stroke of her sword and dispatched another with a well-placed shuriken. Moving with precision and no wasted effort, she executed Jackie Boy himself by slashing his throat, ensuring that the sanctity of Old Town remained intact.
After discovering that Jackie Boy was a police officer, Miho worked with Dwight McCarthy and the Old Town girls to prevent his death from provoking retaliation from the city’s law enforcement. She retrieved his severed head and prepared it for disposal, ensuring no evidence could trace the incident back to Old Town. Later, she helped Dwight transport the bodies to the tar pits, protecting him with her blades as he maneuvered through enemy territory.
During the final confrontation with the mercenaries sent by Wallenquist, Miho struck from the shadows. She used her grappling line to descend upon them silently, dispatching multiple attackers before they could react. Her movements were fluid and lethal; she impaled one man through the chest with her sword, then disarmed another by slicing off his gun hand. Once the threat was neutralized, she stood watch atop a nearby building as Dwight delivered the final blow that ended the conflict.

Personality and attributes
In appearance, Miho was very small, light and a very slim figure, along with long dark hair and dark eyes. She dressed in traditional Japanese-inspired black attire designed for agility, including split-toe boots and a short, sleeveless gi that allowed freedom of motion. Her outfit was often accessorized with her signature weapons — twin katanas and throwing stars — and her expression rarely betrayed emotion. Normally she dressed in a short, modified black kimono worn over a bra, and a belt into which she kept all kinds of weapons. A wakizashi was sometimes shown hanging from her belt. She was often referred to as Deadly Little Miho.
Miho’s personality was defined by her silence and deadly precision. She rarely spoke, communicating instead through deliberate gestures and unwavering stares. Despite her apparent coldness, she displayed loyalty and compassion toward the women of Old Town, acting as their unseen protector. Her violence was never driven by cruelty but by a sense of duty and justice, maintaining the balance of power within Old Town’s fragile peace.

Miho was born an ordinary human with exceptional agility, flexibility, and strength honed through years of disciplined training. Her talents included mastery of multiple martial arts forms, acrobatics, and stealth movement, all refined to the point of near invisibility. She gained her lethal efficiency through personal training and battlefield experience in Old Town, combining traditional ninja techniques with pragmatic urban combat tactics.
