Until 8 years ago, the Chinese military operated the island as a prison with those criminals deemed too dangerous to be incarcerated on the mainland instead being kept within this penitentiary. (Episode: Year's End) At some point, the Chinese military committed a massacre but decided to put general Yao Fei as a scapegoat who was sentenced for life imprisonment on Lian Yu. (Episode: Unfinished Business) Once the program was discontinued, a unit was dispatched to eliminate the remaining residents of the island though two managed to survive. (Episode: Year's End) Edward Fyers intended to set Yao Fei up as a scapegoat for a revolution and had his daughter Shado abducted where she was taken to the island as a hostage to force her father's cooperation. (Episode: Unfinished Business) Operatives from Australian intelligence named Billy Wintergreen and Slade Wilson were dispatched to Lian Yu with the mission to exfiltrate Yao Fei from it. However, Edward Fyers made an offer with Wintergreen agreeing to work with him and betraying Wilson. (Episode: The Odyssey) Fyers plan involved the use of a missile system on the island to destroy a Ferris Aircraft jet heading for China in an effort to cripple the Chinese economy. This was a mission directive assigned to him by his employer and he sought to accomplish it by using Yao Fei as a scapegoat for the plan. This allowed him to be rescued allowing him to return to the United States and Starling City. (Episode: Pilot) Dinah Lance came to believe that her daughter Sara Lance had perhaps survived the sinking of the Queen's Gambit by Lian Yu and possibly was marooned on one of the neighboring islands. (Episode: The Huntress Returns)

During Malcolm Merlyn's plot for Starling City, an artificial earthquake was engineered that led to the death of Tommy Merlyn. Olivier Queen would feel responsible for the best friends death and exile himself to Lian Yu as penance for Tommy as well as failing Starling City. He would only return upon the insistence of Felicity Smoak and John Diggle who urged him back to save Queen Consolidated from a hostile takeover. (Episode: City of Heroes) After defeating Slade Wilson, Oliver had A.R.G.U.S. construct a prison on Lian Yu where he kept Deathstroke imprisoned as his former friend was now depowered of by the Mirakuru serum. (Episode: Unthinkable) Under the threat posed by Ra's al Ghul, Oliver Queen reluctantly allied with Malcolm Merlyn for the eventual confrontation with the League of Assassins. To achieve that end, it was decided that he needed to train and took his sister Thea Queen as well to the island. However, unknown to the pair, Merlyn had the A.R.G.U.S. agent there killed in order to release the captive Slade Wilson. This was because he intended for Oliver and Thea to battle him as part of their 'training'. Ultimately, they were able to defeat Slade and had him imprisoned back in his cell on Lian Yu.
